Car accidents can be overwhelming for many reasons. I aim to make this process as easy as possible for you.
Please inform us upon booking if you have been involved in a recent motor vehicle accident (MVA) so we can ensure enough time.
Your insurance company may provide the information you need, but we have also included some links below to help guide you through the process.
At your first visit after a car accident (whether you are an established patient, or new to our office), please bring your insurance information (company contact, policy and claim #), and your completed AB-1 form. We will submit this form along with the assessment findings. If you choose to go through MVA protocols, your visits will be direct billed by us.
